I HAVE READ THIS WAIVER AND RELEASE OF LIABILITY, AND FULLY UNDERSTAND ITS TERMS.

In consideration of being permitted to participate in the services provided by Regency Cares, I hereby agree to the following:

Acknowledgment of Risks

I understand and acknowledge that participation in the services provided by Regency Cares involves inherent risks, including but not limited to physical injury, property damage, and even death. I voluntarily assume all such risks.

Release and Waiver:

I, on behalf of myself, my heirs, executors, administrators, and assigns, hereby release, waive, discharge, and covenant not to sue Regency Cares, its officers, employees, agents, and volunteers from any and all liability, claims, demands, actions, and causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by me or to any property belonging to me, whether caused by the negligence of Regency Cares or otherwise, while participating in such services or while on the premises where the services are conducted.

Indemnification:

I agree to indemnify and hold harmless Regency Cares, its officers, employees, agents, and volunteers from any loss, liability, damage, or costs, including court costs and attorney fees, that they may incur due to my participation in the services, whether caused by Regency Cares' negligence or otherwise.

Medical Treatment:

I consent to receive medical treatment deemed necessary if I am injured or require medical attention during my participation in the services. I agree to pay all costs associated with such medical treatment and release Regency Cares from any liability for such treatment.

Governing Law:

This waiver and release shall be governed by and construed in accordance with the laws of the state of Colorado.

Severability:

If any provision of this waiver and release is found to be unenforceable or invalid, the remaining provisions shall remain in full force and effect.
